Our policy for the use of patient stories is intended to protect our patients and maintain a respectful and positive website.

A signed consent form giving us permission to publish your story must be on-file before a story may be published.

A patient may request in writing that their story be removed. All such request will be honored. 

We will not consent to the use of any patient information beyond its publication on our website in any shape, way, or form, without first obtaining written consent for any such additional use from the patient.

We will not publish any content if we believe it is about or by a legal minor.

Any information that is negative, and/or may cause harm and/or infringe or violate the rights of any party will not be used.
